This will be a cloud-based technology platform for remote patient monitoring and engagement at virtual wards. It enables personalised virtual care through assigned care pathways predicting risk scores through vitals and questionnaires and through specially tailored educational content for encouraging patient engagement during their treatment. Each of the assessment questionnaire is being assigned with a specific score based on which risk score is being generated based on the answers from patients.

If a respondent selects "yes" to a specific question, additional relevant questions will follow, while a "no" response will allow them to proceed to the next section. The survey should be easily accessible via email invitations or be completed directly by operators, ensuring flexibility in data collection.

The system should support data retrieval, reporting, and potential integration with marketing automation tools, making it a scalable and user-friendly solution for consumer engagement.
